Who is responsible for your data?

What the consultation form collects
We use your name, telephone number, optional email address and preferred clinic to handle your request and arrange a consultation. We also record that you used the English form. The legal basis is Article 6(1)(b) GDPR: taking steps at your request before providing a service. A name and telephone number are required to handle a request through this form; you can also call us. Receipt of a request does not confirm an appointment. This English form does not ask for medical records, symptoms or other clinical information. You can discuss these at your consultation.
Who can access the information?
Your request is not public. An authorised doctor can access it after signing in to the private patient workspace. OpenAI Sites provides the hosting, with Cloudflare D1 database infrastructure. Brevo handles email notifications and receives the recipient address and notification text. Clinical questionnaire answers are not included in these notifications. Technical providers may access information as necessary to operate and support the services. Your request is not sent to the public chatbot and is not used to subscribe you to advertising.
Where the data is processed
The hosting service does not guarantee processing exclusively within the EU. Under the OpenAI Sites data processing addendum, transfers outside the EEA rely on an adequacy decision or standard contractual clauses. How long the data is kept
We keep your request while handling it, arranging the appointment and managing related communication. Once the purpose ends, unnecessary working data is removed; there is no automatic deletion after a fixed number of days. If relevant information becomes part of a medical record, the healthcare provider’s separate statutory duties apply. A website enquiry is not automatically retained as a medical record. Provider notification emails, operational logs and backups have their own lifecycle.
Your rights
Subject to the conditions of the GDPR, you can request access, rectification, erasure, restriction of processing and data portability. You may object to processing based on legitimate or public interest. Send requests to the contact email above; we will verify your identity before providing personal data. You can complain to the Office for Personal Data Protection of the Slovak Republic at https://dataprotection.gov.sk . No automated system decides on your treatment or acceptance as a patient.
After you send a request
A necessary secure cookie lets you return to the same request in this browser for up to two hours. On a shared device, use “End access on this device”. This closes browser access without cancelling your request. If you provide an email address and email delivery is available, you may receive a receipt. A notification failure does not delete a saved request. Do not email medical records in reply to the receipt. This form is not an emergency service.
